The pads squeeze the disc when you press the pedal: the friction between the two is what stops the car. The friction material wears down, and underneath it is the steel backing plate, which damages the disc.

You look at the thickness: below 2–3 millimetres of material they are replaced. The signs are the wear warning light, a metallic squeal when braking, or a pedal that goes down further than usual.

They are always replaced per axle, both wheels together. Front and rear are different parts, and the system matters: caliper size, wear sensor, make of the braking system. On a car that is a few years old it is worth checking the discs at the same time: new pads on a worn or scored disc brake worse and last less.
